• Step1 -

  Right Click Here and "Save As." to download your completed social security card application.

• Step2 -

  Sign section 17 of the completed SS-5 form and enter your SSN with BLUE or BLACK ink in Section 2

• Step3 -

  Prepare supporting documents:

  Accepted are only certain original or issuing agency certified copied documents as proof of identity. Documentation must be valid (not expired) and show your name, identifying information (date of birth or age) and preferably a recent photograph. For example, as proof of identity Social Security must see your:

  • U.S. driver's license
  • State-issued non-driver identification
  • U.S. Passport

  If you do not have one of these specified documents or you cannot retain a replacement for one of the above listed documents within 10 days, we will ask that you provide one of the following documents:

  • Employee ID card
  • School ID card
  • Health insurance card (not a Medicare card)
  • U.S. military ID card

  Specifications:

  • Applying for an Original Social Security Number for a U.S Born Adult (Age 12 or Above): In addition to the above listed steps, you must provide your birth certificate and appear for an interview at a Social Security Office.
  • Applying a with a name change: If you need to change your name on your Social Security card, you must provide a recently issued document as proof of your legal name change. Documents Social Security may accept to prove a legal name change include:
  • • Marriage document
    • Divorce
    • Court order for a name change

    You must provide at least two separate forms of documentation.

• Step4 -

  Submitting documentation (You must mail in the application to final the process)

  • In most cases, you can take or mail this signed application with your documents to any Social Security office. Any documents you mail to us will be returned to you. Go to https://secure.ssa.gov/apps6z/FOLO/fo001.jsp to find the Social Security office or Social Security Card Center that serves your area.
